Online Referral Form

Thank you for considering Evita Social Housing CIC.

This form can be completed by professionals, organisations, family members, or individuals wishing to refer themselves for supported accommodation in Birmingham or Coventry.

Evita provides safe and stable accommodation alongside structured, person-centred support to help individuals develop independence, maintain their accommodation and work towards longer-term housing.

Before you begin:

The applicant must be 18 or over Be in receipt of Benefits They should have a identified support need They must be able to manage their own personal care independently They should be willing to engage with regular support

The form should take approximately 5-10 minutes to complete.

Once submitted, our team will review the referral and contact you to discuss the next steps. Submitting a referral does not guarantee an offer of accommodation and all referrals are subject to eligibility, needs and risk assessment.
